Big Teams: Process for turning in Athletic Physicals and Paperwork
Attention athletes and parents! We are teamed up with Planet HS/BIG Teams to stream line the physical process and athletic paperwork. Please click below to start creating accounts. Both parents and students must create an account and link them together. This will be the only way athletic paperwork will be taken.
Physicals must be dated after April 1, 2024 in order to be valid for the 24-25 school year. Please use all online forms when filling out information.
Only two items that should be uploaded.
1. Doctor’s Physical Exam dated after April 1, 2024 with doctor signature
2. Birth Certificate
